Mission
The mission of the Transitions Program is to provide case management and volunteer services to individuals with a life-limiting illness.
Services
The Transitions Program provides individual/family support, volunteer services, monitors the changing needs of the client/family, and coordinates community services as appropriate.
Volunteers provide friendship, socialization, light housekeeping, food preparation, assistance with errands, and provide the family a respite break.
Who is Eligible?
- The individual lives in Hillsborough County
- The individual has been diagnosed with a life-limiting illness
- The individual is eligible regardless of the client`s current medical treatment, whether it is curative treatment or palliative treatment
- The attending physician provides an order for the program
- No individual shall be refused service due to race, creed, color, sex, age, national origin, religious background, physical disability, sexual orientation, veteran status, financial status, medical treatment or the ability to pay
