Admission Criteria
LifePath Hospice admits patients diagnosed with life-limiting illnesses who have a prognosis of one year or less. Though most hospice insurance benefits only cover services for the last six months of life, individuals who meet LifePath Hospice's admission criteria will receive care regardless of their ability to pay. Admission Criteria
- Patient is diagnosed with a life-limiting illness
- Patient's prognosis is one year or less - if the disease takes its natural course - as confirmed by an attending physician
- Patient is primarily interested in a palliative plan of care
- Patient and family desire LifePath Hospice services
Referral Information
Usually patients are referred to LifePath Hospice by their primary care physician. However, the process can be initiated by other health care professionals, family members, friends, clergy or patients themselves.
