LifePath Hospice brings strength, understanding and comfort to the families we serve. We empower patients to make the most of life as they journey toward its natural conclusion.

LifePath Hospice helps patients and their families by becoming a multifaceted support system, arranging every service needed in regard to the illness.

Our care is delivered through a highly qualified team of physicians, skilled nurses, certified home health aides, clergy, social workers, counselors and volunteers. This team approach provides families with an invaluable resource, providing a truly encompassing level of services that supports physical, spiritual, social and emotional needs.

And since hospice is about the quality of life, LifePath Hospice care is primarily provided in the home, where patients have the loving support of their family and friends. As the definition of “home” may differ from patient to patient, our services extend to those residing in hospitals as well as long-term care facilities including nursing homes and assisted living facilities. We can even help with the transition into different settings as needed.

  • Our expertise in pain control and symptom management enhances the patient’s ability to make the most of life by keeping patients as PAIN FREE AND COMFORTABLE as possible. We remain just a phone call away through our hospice team, which is available 24 hours a day, seven days a week.


  • Family support is key. Our services enhance the family’s ability to keep their loved one in the comfort of their own home. Home health aides EASE THE FAMILY BURDEN by assisting with daily tasks such as bathing, light cooking and assistance with household chores.


  • LifePath Hospice provides families with the tools they need as they journey with their loved one through advanced illness. In-depth PATIENT CARE EDUCATION and training for caregivers is available to help families succeed in the caregiving role.


  • HOME VISITS from physicians, nurse practitioners and other hospice /professionals continue through the cycle of care.


  • LifePath Hospice’s trained volunteers connect with patients and families to provide companionship and support. Volunteer support comes in many degrees to aid EACH FAMILY'S INDIVIDUAL NEEDS. Whether it’s a simple gesture like holding a patient’s hand, reading from their favorite book, providing music or pet therapy, assisting with grooming, or running a much needed errand, our volunteers profoundly enhance the lives of our patients and their families.


  • Though most families prefer to keep their loved one at home, we know the demands that accompany this choice. LifePath Hospice provides RESPITE SERVICES FOR FAMILY CAREGIVERS, allowing time to regenerate, refocus and attend to other responsibilities.


  • LifePath Hospice brings comfort to those facing an advanced illness by providing SPIRITUAL CARE to patients and families.


  • CAREGIVER ASSISTANCE is available during brief periods to support patients who may have a frail caregiver, or no caregiver at all. Caregivers perform home-making and personal care tasks to help the patient live more safely and comfortably in their own home.


  • There may be times when a patient’s care needs become too difficult to manage at home. Our HOSPICE HOUSES are acute inpatient facilities designed to provide a warm, homelike environment for patients and their families during these times. LifePath Hospice staff and volunteers are on hand to provide care and service with compassion and understanding.


  • Our family support continues after the patient’s death through our BEREAVEMENT CARE. We guide families through the grieving process from understanding to acceptance and forward on to healing.

HospiceHelp24 is our 24-hour help line, offering both scheduled and urgent care visits to patients and families around the clock, as well as after-hours telephone assistance to address all types of questions and concerns. After-hours staff includes: physicians/nurse practitioners, nurses, chaplains, social workers, pharmacists and equipment technicians. HospiceHelp24 is not an answering service.
