How to Be Admitted

LifePath Hospice admits patients who have been diagnosed with life-limiting illnesses and have a prognosis of one year or less. Though most hospice insurance benefits only cover services for the last six months of life, if individuals meet LifePath Hospice's admission criteria, they will receive care regardless of their ability to pay. Please contact a representative at the number listed below for more information about your private insurance and Medicare benefits.

Admission Criteria
  • Patient is diagnosed with a life-limiting illness
  • Patient's prognosis is one year or less - if the disease takes its natural course - as confirmed by an attending physician
  • Patient is primarily interested in a palliative plan of care
  • Patient and family desire LifePath Hospice services
